Thank you to all who participated in our iPad2 Contest which concluded on May 8, 2011. Congratulations to all of our 72 winners this year. All of our winners are listed below.

Celebrate the grand opening of our newest store at 'Upper Canada Mall' in Newmarket, Ontario!!

Enter to win an iPad2 plus $1000+ worth of other prizes. For a chance to qualify; simply earn a ticket for every $100 spent (tax excluded) in any Eyestar location from now until May 8th.

On May 9th, our draw will commence: Two lucky contestants will win an iPad2 - one in ON and one in BC along with an additional 10 winners per store whereby each winner receives a $50 gift certificate!

1. ELIGIBILITY AND PARTICIPATION

- This contest is open to anyone currently residing in Canada, except employees of Eyestar Optical Group, its contest sponsor, advertising and promotional agencies and their respective affiliates and associates and such employees' family members.
- Contest applies to prescription and sunglass frames only, excluding contact lenses.
- Contestants are responsible for compliance with applicable laws in jurisdictions where they reside.
- Winner must answer a simple skill testing question.
- All personal information given, is the property of eyestar, and will not be sold or given to any 3rd party.
- All participants must be over age of 19.

2. SUBMISSIONS OF ENTRIES AND DEADLINES

- All entries must submitted no later than 7pm PST, May 8th, 2011. Late entries will be rejected.
- To be eligible to participate, entrants must provide their name, valid email address, and all other required information.
- Incomplete entry forms will be deemed disqualified and will be rejected. Eyestar Optical is not responsible for any lost, misdirected or delayed entries. There is also no responsibility or liability for any potential entrant's inability to access the official Contest submission function for contest entry. Entries received by telephone, fax, courier, personal delivery or Customer Service Ticket or any other method other than earning tickets from purchasing will not be accepted.
- Notification of Winners. Winners of prizes shall be notified immediately following the announcement of the winners on the eyestar.ca website on May 10th, 2011. As a condition to receipt of their prize, prize winners are required to show the winning ticket and proof of identifications.

3. PRIZES

- Grand Prize. The Contest grand prize winner will win a brand new iPad2 from Apple.
- Consolation prize. There will be 10 consolation winners from each store who will receive a $50 gift certificate from any of the Eyestar Optical locations.
- In accepting the prizes, the contest winners acknowledge that Eyestar Optical may not be held liable for any loss, damages or injury associated with accepting or using the prize(s).
- Notification of Winners. Winners of prizes shall be notified immediately following the announcement of the winners on the eyestar.ca website on May 10th, 2011. As a condition to receipt of their prize, prize winners are required to show the winning ticket and proof of identifications.
- Eyestar Optical reserves the right to publish the name, location and a recent photograph of the Winning Entrants in any medium without remuneration.
- All entries become the property of Eyestar Optical Ltd.
- Decisions of Eyestar Optical Ltds are final.
